What information do we collect?
We collect 2 kinds of information from visitors to www.hsf.eu.com: Health Cash Plan Application information and site usage information from session cookies and log files.
Health Cash Plan Applications
If you make an application for a Health Cash Plan through the www.hsf.eu.com website, we collect 3 types of information: your personal details (including those of your partner and any dependants), your Medical details (including those of your partner and any dependants) and payment details.
- Personal details
The personal details we collect are: your personal and contact details including name, address, date of birth, company name and address (if applicable), email address and telephone numbers. We also collect the name and date of birth of your partner (if applicable) and any dependants (if applicable). - Medical details
The medical details we collect are: any conditions or illness you, your partner and any dependants may have had (or have) and the date any symptoms began.
Personal and Medical details are stored to allow us to approve your application and send you relevant information relating to it. A copy of this information is kept securely by HSF and our technology suppliers, Lumison and Whitespace. - Payment details
The payment details we collect are either Direct Debit information or credit/debit card information. Direct Debit information will be used for automatic payments to be made from the bank account you provide. A copy of this information is kept securely by HSF (and temporarily by our technology suppliers, Lumison and Whitespace).
Credit/debit card information will be used as a financial transaction for your initial online purchase, but will not be retained after successful authorisation of the transaction. For your security, authorisation for credit/debit card purchases will be handled by online payments provider www.protx.com and checked with the credit card company before confirmation of any bookings are made. In addition to our credit card policy, our secure server software (SSL) is the industry standard and among the best software available today. It encrypts all of your personal information, including credit card number, name and personal details so that it cannot be read as the information travels over the Internet.
Personal details, medical details, and direct debit information is also stored on back-up tapes and discs to avoid loss of data. Such data will be held until you inform HSF otherwise. In such circumstances your registration will immediately cease.
Site usage information – log files
Log files allow us to record visitors' use of the website. Lumison compiles a log file of information from all visitors to www.hsf.eu.com. This information is used to inform changes to the layout of the site and to the information within it based on the way that visitors move around the site. Log files do not contain any personal information.
