Find a Practitioner

As an HSF health plan policyholder, our schemes offer you help towards the cost of consultation and treatment by a qualified and registered practitioner. Although we are not able to recommend any specific practitioner, to assist you in finding someone suitably qualified, please find a list of websites and professional bodies below.

Any practitioner you choose must be able to supply evidence that they have the appropriate qualifications and insurance to provide the treatment being covered. If you are unsure, please call us on 020 7202 1381.

Making a claim

Keep the receipt showing the payment to the practitioner. Download and fill in a claim form – details here.  The claim form should be returned with any relevant receipts.
