This plan covers you, your partner and dependent children (up to the age of 18) living with you.
If you have pre-existing health conditions, please read the brochure and Terms and Conditions to check our terms.
These plans start from £7.75/month (Scheme P1) up to £94.50/month (Scheme PD).
All plans include HSF Assist which provides 24/7 GP Advice Line, Virtual Doctor, Counselling Service and Legal Advice Line.
All plans include HSF Perkbox, a discounts service covering discounted gym membership, shopping discounts and days out.

This plan covers you and dependent children (up to the age of 18) living with you. A partner can also join a scheme at the same rate.
The plan provides cover for pre-existing health conditions. Please read the brochure and Terms and Conditions for more detail on this.
These plans start from £5/month (Scheme V1) up to £70/month (Scheme V7).

Your Bank Details.
The names and dates of birth of everyone to be included on your plan.
Any medical information requested concerning everyone included on your plan. This is not required on HSF Individual as this plan covers pre-existing conditions.
